Best practice in the transition to adult services for young adults who had childhood cancer

Abstract

This article highlights the importance of nurses supporting young adults who had a childhood cancer to prepare for the transition from children’s to adult services. Preparation of the young adult, their families/carers, children’s and adult services is crucial for a successful transition to adult services. A poor transition to adult services risks loss to follow up, however, a well organised person-centred transition plan commenced early will contribute to a young adult who feels ready to transfer to adult services and engages with adult services. Nurses in both children's and adult services are jointly responsible for preparing the young adult to successfully transition to adult services.
