Dental Extractions Before Head and Neck Radiotherapy: A Delphi-survey

Abstract

Objectives: To improve the clinical decision-making process regarding pre-radiotherapy dental extractions for head and neck cancer (HANC) patients.

Methods: Three separate questionnaires consisting of both open- and close-ended questions or statements, were emailed to an ‘expert panel’ of Restorative Dentistry Consultants (clinical specialists) based in the United Kingdom (UK) and Ireland over three Delphi iterative rounds (n=24/28; 86%). Each questionnaire was designed after analysing the results of the previous round(s) and a thorough review of the relevant literature. In Rounds 2 and 3, participants were asked to indicate on a 5-point Likert scale their level of agreement with a series of statements regarding the types of teeth that should be extracted prior to radiotherapy. The target level of consensus for each statement was 70%.

Results: In Round 2, there was clinical specialist consensus agreement for 69 of the 102 statements (i.e. greater than or equal to 70% of participants rated ‘agree’ or ‘strongly agree’ for each statement). Consensus agreement was also achieved for 20 out of 28 statements in Round 3. Therefore, a total of 89 consensus statements were used to illuminate the decision-making process for the pre-radiotherapy dental extraction of molar, premolar, and anterior teeth with periodontal pocketing, furcation disease, mobility, dental caries, tooth-wear, apical disease, or other associated pathology, in patients with HANC.

Conclusions: The compilation of consensus statements represents the majority view of Consultants in Restorative Dentistry in the UK and Ireland regarding which types of teeth should be extracted prior to radiotherapy in HANC patients. The results of this study provide a firm platform for the development of future clinical guidelines for pre-radiotherapy dental extractions.
