Abstract

This workshop will bring together clinicians, researchers and the voluntary sector to learn, collaborate and most importantly honour one of our stroke survivors and families most important requests. The request to improve transitions of care across the stroke pathway so that they do not feel abandoned, are supported to maximise their recovery, understand and manage their cardiovascular risk factors and ultimately maximise their long-term health and functional outcomes. This will involve multiagency working across traditional boundaries, changing the mindset of clinicians and the implementation of complex behavioural change interventions which empower stroke survivors and their families. The time for accelerating improvements in post-acute care is now.
