Abstract

Optimising delivery of palliative care within care home settings: The FSHC Palliative Care Supportive Framework
Background:
For some time there has been a pervading notion amongst practitioners that palliative care is usually only appropriate during the terminal stages of illness, most notably cancer. Within care home settings nurses and care assistants occupy an important role in relation to provision of palliative care because diseases like dementia, heart failure and COPD can commonly affect the older person.
Intervention:
Four Seasons Health Care (FSHC) have developed a palliative and end-of-life care supportive framework (PCSF) which serves to enable nurses and care assistants within our care homes to deliver optimum levels of palliative care. The PCSF was developed with consideration given to best practice recommendations, recent empirical evidence and expert commentary. It was disseminated to 80 care homes across Northern Ireland and Wales through production of a new company policy, development of an extensive topic manual and roll-out of mandatory e-learning module.
Framework:
The PCSF provides care staff with advice on a range of topics including: difference between palliative and end-of-life care, how to utilise the SPIKES framework for breaking bad news, how best to host family conferences, developing advance care-plans in collaboration, using the gold standards framework and how to provide care during and after death.
Conclusion:
The PCSF has been recently launched and now outcome measurements are available. That being stated, the PCSF has provided our care staff with a new understanding of palliative care which recognises that, as the Prague Charter asserts, it is a human right for all people living with incurable illness.
