Abstract

This chapter presents and explores definitions and theories related to transitions and the policy and practice responses necessary to ensure the needs of individuals at different transition points are identified and met effectively. The complexities of different transition processes and the implications on the care and support of people with intellectual disabilities are considered with a specific focus on healthcare that is linked to a research study undertaken in Scotland. A case vignette of the transition needs of Sarah, a young adult with intellectual disabilities, will be presented, highlighting the care and support needs necessary to ensure her effective transition.
